All Waverly Student-Athletes grades 7-12 MUST have a completed FinalForms Account to practice & compete. This includes an uploaded copy of a current, doctor completed & signed MHSAA Physical Form releasing the student for participation. Update your FinalForms at the beginning of each school year by marking the Sports you are interested in.
A CURRENT-YEAR PHYSICAL IS ONE GIVEN ON OR AFTER APRIL 15 OF THE PREVIOUS SCHOOL YEAR

Locate parent icon and click NEW ACCOUNT
Type your NAME, DATE OF BIRTH and EMAIL
Click REGISTER
Create a new FinalForms password and click CONFIRM ACCOUNT
Click REGISTER STUDENT for your first student-athlete
Locate and click the ADD STUDENT button
Type in LEGAL NAME and other required information
Click CREATE STUDENT
Check boxes the student-athlete is interested in
Click UPDATE after making selections
Complete each form by signing your full name electronically at the bottom and click SUBMIT FORM
